  • Skilled Nursing Care - Registered Nurses and Licensed Practical Nurses.

  • Wound Care - Provide specialized care in recovery from wounds.

  • Medication Management/IV Therapy - Nurses will evaluate the patient’s medications with each visit.

  • Therapy - Physical, occupational and speech therapy

  • Vital Stem Therapy - Provided by Speech Therapists for patients with difficulty swallowing.

  • Medical Social Services - Licensed social workers

  • Home Health Aide - Certified Nursing Assistants assist with personal care needs.

  • Dietitian - Nutritional services available through Registered Dietitian.

  • Telehealth Services - Available for CHF/COPD and any other high-risk diagnosis. Through our Telehealth Services we are able to monitor patient’s vital signs, weights, blood glucose levels, and symptoms on a daily basis. TruCare can perform Telehealth 2-way visits with our patients on an as needed basis.

  • Palliative Home Health - TruCare now offers Palliative Home Health through our TRAC Program. Transition Readiness for Advanced Care is designed for patients with advanced disease processes who are still seeking curative measures. 

According to Federal law, home health care patients are free to choose which institution, agency or person will provide their home care services.

Section 1802 of the Social Security Act "seeks to ensure that free choice is guaranteed to all Medicare Patients." The law states: "Any individual entitled to insurance benefits under this title [i.e., Medicare] may obtain health services from any institution, agency, or person qualified to participate under this title if such institution, agency, or person undertakes to provide him such services." This gives patients freedom to choose whomever they want as their provider of post-hospital services.
